For Adult with
Disabilities - Continuing Education Center with a Real Difference.
Country Club Day
Activity Center provides a week-day organized program for adult
basic skills and services. It is in a community group setting that
we support personal independence and promote the social, physical and
emotional training, and well-being for the physically/mentally
challenged individuals in our care. It is our desire to help
educate people with developmental disabilities by giving them the
opportunity to choose and realize their goals of where and how they
learn, live, work, and play.
Services include a variety of group discussions, classes, activities,
health monitoring, help with activities of daily living,
intergenerational programs, individual and family counseling, recreation
therapy, bathing and grooming, physical exercise and nutrition. These
services are designed to assist people with developmental disabilities
live successful, meaningful lives. We believe that all people should
have access to a quality lifestyle in the community and our supports are
individually designed to assist people in achieving that lifestyle.

Staff
We offer sincerely caring and well trained professional staff which
enable us to keep our participants as mentally and physically active as
permissible. Fostering a positive image and role model to each member of our
program is a core principle at CC4D. All staff are provided specific training to our
particular program and are trained to work effectively with
physically/mentally challenged individuals. Our staff receive
continuous on-site training and are encouraged to take full advantage of
many off-site training development opportunities as well.
Eligibility Criteria
Participations must be at minimum 17 years of age or have written
release from the public school system. Participants include adult
individuals of all ages with MH, DD or SA. Members participation
may be on a routine scheduled or drop-in basis. Participates
includes both Ambulatory and Non-Ambulatory special needs adults.
Every effort is
made to arrange safe, reliable transportation - in some cases it is
provided by the Center and in other cases by the family or town of
residence. |
